I'm not suggesting that you should -- but my plugins / search engines / bookmarks somehow became corrupted, and I needed to uninstall and reinstall. The uninstall engine (by design, I assume) does not get rid of these settings, so if you need to completely nuke Firefox from your system, find this directory:
C:\Documents and Settings\[YOURNAME]\Application Data\Mozilla
And delete it. The "Application Data" folder may be hidden, so you'll need to go to Windows Explorer and hit Tools > Folder Options > View and hit the "Show hidden files" radio button.

Better yet, you can do the following:
Run from the command line (or Start-->Run) your\path\to\firefox.exe -profilemanager
Then create your personal profile in a directory with a more sensible location and name (I'm partial to "default" on machines where I'm the sole user). Then when this happens, you can 1) regularly back it up so as to restore it if corruption occurs or 2) selectively delete things from time to time.
What I want to find is the document where Fx keeps your declared cookie exceptions, so that I can tailor them. I like how they now show you your cookies by domain, but the exceptions are in pure alpha order.
